The registration process for the London Pharmacy MenACWY Vaccination Service 2018 is now open

This London service covers only the following vaccinations by Community Pharmacists:

Meningococcal group A, C, W and Y conjugate vaccine (MenACWY - Nimenrix® Brand).

Pharmacies registering to provide this London Pharmacy MenACWY Vaccination Service 2018 will need to register as a provide below in order to provide this service and for the payments to be made. Furthurmore, one lead pharmacist or manager must have signed a hard copy of the correct SLA & Service Specification on behalf of the pharmacy which should be kept on file in the pharmacy (strictly not to be sent to anyone!)

Each Pharmacist providing this service must have signed a hard copy of the relevant PGD, again to kept on file in the pharmacy (strictly not to be sent to anyone!)

Declaration on behalf of the pharmacy

You undertake on behalf of the pharmacy and the pharmacist(s) delivering the service to answer the following questions and declarations correctly:
- Please confirm your pharmacy can meet: (To register - all answers must be ticked)
  - Has indemnity insurance to provide vaccination Services
  - Has access to web-based Sonar platform to ensure patient vaccination data is transmitted electronically via Sonar to GP within 24 hours (or 48 hours at weekend)
  - Has an appropriate sized consulting room, with hand washing facility near by to accommodate parent and child and to manage incidents safely
  - Pharmacists meet the training and accreditation requirements as per SLA
  - Proactively offer this vaccination service to the target group 18 - 25 year olds.
